A Home Care Package provides a coordinated package of services designed specifically for you.
There are four levels of this service:
Thomas Holt provides all Home Care Packages on a Consumer Directed Care (CDC) basis. This means that you have the choice as to what types of services you receive and how that support is delivered to you.
You can choose how much involvement you would like in managing your own package of services. You will be provided with a personalised budget and see how much funding is available to you and how that money is being spent.
The Australian Commonwealth government funds these packages, but you will be asked to contribute to the costs.
If you are on a pension, your maximum contribution is 17.5% of the single aged pension. People on higher incomes may be asked to contribute a higher amount.

If your needs indicate you may need this kind of support, we can refer you for an assessment by the Aged Care Assessment Team (ACAT) to determine if you are eligible or you can personally register with My Aged Care on 1800 200 422 or www.myagedcare.gov.au.
